Step 1: Referral
Every admission begins with a pre-admission assessment. Typically, the Case Manager will contact one of our Rehab Admissions Liaison’s to request a pre-admission screen to be completed. You, or your family member or physician will also meet or speak to one of the Rehab Admissions Liaisons. The liaison will provide you with information about our facility and explain what it means to be a patient at our inpatient rehab facility. Together, we will discuss goals for recovery and review current medical and therapy information to determine the most appropriate treatment needs and options.
Family members or other representatives are invited to tour our inpatient rehab unit to see our facility for themselves, while meeting a few of our healthcare providers. Our admissions staff will check health insurance benefits too, so you can make an informed choice about this critical part of your recovery.
Step 2: Admission
The criteria for admission is determined by Medicare and other healthcare regulatory organizations. All admission determinations are made by a team of healthcare professionals, led by our Medical Director of Rehabilitation. As part of our pre-admission process, you will meet or speak to one of our Rehab Admissions Liaisons who will provide you with information about our facility and will explain what it means to be a patient at our inpatient rehab facility. We will discuss goals for recovery and review current medical and therapy information to determine treatment needs and options.
Step 3: Discharge
The discharge planning process begins soon after the patient admitted and involves the patient, family members and rehabilitation team. Individual progress and needs are addressed at the team meetings, family education sessions and family conferences. The case manager and therapists will work with you to help arrange for the necessary home medical equipment, follow-up services, and community resources upon discharge.
